Thank you for joining the NuLook Home Care family. This onboarding portal is designed to ensure we have all the required documentation and acknowledgments in place to support your employment and maintain compliance with state and federal regulations.

Whether you are joining us as a CNA, PCA, LPN, RN, or an administrative professional, this step-by-step process will help you complete your employee file with ease.

What You’ll Complete Today

Depending on the position you selected in your application, you may be required to complete some or all of the following:

✔ Employment Application (if not already submitted)

✔ Direct Deposit Authorization

✔ Federal W-4 Form

✔ I-9 Employment Eligibility Verification (W-2 employees only)

✔ NC-4 Form (for employees only, not subcontractors)

✔ W-9 Form (for subcontractors only)

✔ Background Check Consent

✔ HIPAA & Confidentiality Agreement

✔ Handbook Acknowledgment

✔ Policy Acknowledgments (Dress Code, Safety, EVV, Incident Reporting, etc.)

✔ Credential & ID Verification (based on position)

✔ Proof of Active Certification (e.g., CNA License, RN/LPN License, CPR Card)

✔ Valid Driver’s License (if providing transportation or traveling to clients)

If you're joining as a subcontractor, you'll skip the I-9, W-4 and NC-4 forms but must complete the W-9 and provide your business credentials or professional license where applicable.
